• The drivers license (or state ID) and social security number of anyone authorized to sign on the account,
• A check with the minimum opening deposit.

2. Change Your Automatic Payments

Print the form below and give it to the companies debiting and/or crediting your old account.

3. Close Your Old Account

Make sure any outstanding checks have cleared your account and you have notified any companies initiating automatic payments with your new account information (using the above form). Then send your old bank this form to close your account.
